ABOUT THE ROLE
The Case Coordinator-CHSP is part of the Aged Care team and is responsible for effectively coordinating the activities of SWMS’s Senior Social Support Groups, which are part of the CHSP Program. This role’s key purpose it to ensure that all of SWMS’s Senior Support Groups receive exceptional customer service through coordination of meaningful activities, events, and other relevant projects within the Aged Care Team. The CC-CHSP is also responsible for compliance of the Social Support Program under the Aged Care Act and the Aged Care Quality Standards as well as ensuring that quality services are delivered to all consumers. This is full-time, fixed term until 30/06/2027.
