PURPOSE OF THE ROLE
The Project Support Officer - CHSP is part of the organization’s Aged Care Division. The main purpose of the role is to provide exceptional administrative and compliance support to the Aged Care Services Social Support Program with a strong focus on compliance and group engagement and monitoring. The role will effectively assist the Case Coordinators, CHSP & Team Leader, CHSP in all tasks required for the intake, reassessment and reporting of Social Support Group clients. The role will assist with administration tasks such as scanning, client management system data entry and other administration support as requested daily. The role will communicate via various communication tools ensuring that all administration tasks are completed accurately, at a high standard and in a timely manner.

Job Description
DUTIES & RESPONSIBILITIES
Setting-up rooms for group meetings and sessions relevant to the need of the group.
Collect all attendance sheets after every meeting.
Administration tasks in the Client Management System.
Coordinate outings ensuring name lists, itineraries are organized for each outing, collecting funds & issuing receipts.
Monitor the right amount of funds as prescribed by the organisation at any point in time is collected; Liaise with the finance team to ensure recording of funds.
Monitor & organize the stock for the groups & in the Storeroom.
Assist and conduct client reviews, follow-ups & referrals & ensure that reviews are aligned with organisation procedures, meet guidelines and compliance.
Prepare monthly calendars of activities and distribute to groups.
Retrieve & transfer all relevant MAC documentations into the U drive & the Client Management System for the Social Support group clients.
Assist with various admin tasks involved in organizing & coordinating events throughout the year.
Provide additional assistance as required on days where there are more than 2 groups meeting within the building.
Provide other tasks as requested such as obtaining Community transport MAC referral code for all social support group clients, raise invoices, assist clients in and out of transport and assist with ordering food.
Provide a trusted and effective conduit of information within the team.
Keep up to date with contemporary knowledge of aged care sector reforms including understanding of the principles and guidelines.

Maintain accurate individual records, data and system compliances.

Monitor the physical, mental, and emotional conditions of consumers and reporting any changes to Case Coordinators, CHSP.
Provide reports in a timely manner as requested by relevant supervisors.
Desired Skills and Experience
ESSENTIAL CRITERIA
High level administration skills
Understanding of aged or community services and/or relevant experience.
High levels of initiative, confidentiality and sensitivity in the performance of work.
Computing skills, including basic operating systems; managing databases.
Proven ability to work autonomously and with minimal supervision with great initiative.

Understanding of how this role relates to the outcomes of the organisation’s strategic objectives.
Excellent interpersonal and customer focused skills with the ability to engage and build trust and rapport with a diverse range of people.
Understanding of the issues faced by elderly people from a CALD background in accessing services.

Proven record of working effectively with CALD communities and understanding of their barriers in accessing services.
